Registered Nurses
(RN)
Full Time, Per Diem, and Weekends
Current Pennsylvania RN license and CPR certification. Responsible for providing skilled nursing care to ill patients in their homes; recording clinical and progress notes; and coordinating other patient care activities/services to provide the highest quality of patient care. Must possess knowledge in assessing, implementing and evaluating client response to actual/potential medical problems. Ability to work flexible hours as required meeting identified patient needs and/or changing organizational needs. Excellent communication, organization and interpersonal skills needed.
Licensed Practical Nurse
(LPN)
Current Pennsylvania LPN license; CPR ce.rtified. Must have home care experience with a minimum of one year of acute care nursing experience and possess strong patient assessment, patient management and documentation skills. Responsible for providing direct nursing care as prescribed by the physician to ill patients in their homes; recording clinical and progress notes; and coordinating other patient care activities/services to provide the highest quality of patient care under direct supervision of RN and/or designated supervisor. Nursing activities are limited to these in compliance with the State LPN/LVN Practice Act. Excellent communication, organization and interpersonal skills needed.
Home Healthcare Aide
(HHA)
Completion of state-approved HHA training program and/or successful passing of a competency evaluation program which meets state and federal guidelines. Responsible for providing various client-related services to meet the personal care needs and to promote comfort and independence to ill patients in their homes; and recording clinical and progress notes. He/she works under the direct supervision of the designated supervisor.
Personal Care Aide
(PCA)
Completion of PCA training program (40 hours) and/or successful passing of a competency evaluation program which meets company guidelines. Responsible for providing various client-related services to meet the personal care needs and to promote comfort and independence to ill patients in their homes; and recording clinical and progress notes. He/she works under the direct supervision of the designated supervisor.
Physical Therapist
(PT)
Current Pennsylvania PT license or registration; CPR certification. Minimum of one year of acute care experience required; previous home care experience preferred. Must possess strong patient assessment, patient management, and documentation skills. Responsible for providing skilled therapy care to ill patients in their homes; recording clinical and progress notes; and coordinating other patient care activities/services to provide the highest quality of patient care. Ability to work flexible hours as required to meet identified patient needs.
Occupational Therapist
(OT)
Current Pennsylvania OT license and Registered by the Occupational Therapist Association, as well as current CPR certification. Minimum of one year of acute care experience required; previous home care experience preferred. Possess strong patient assessment, patient management, and documentation skills. Responsible for providing skilled therapy care to ill patients in their homes; recording clinical and progress notes; and coordinating other patient care activities/services to provide the highest quality of patient care. Ability to work flexible hours as required meeting identified patient needs.
Speech Therapist
(ST)
Current Pennsylvania ST license and CPR certification. Minimum of one year of acute care experience required; previous home care experience preferred. Possess strong patient assessment, patient management, and documentation skills. Responsible for providing skilled therapy care to ill patients in their homes; recording clinical and progress notes; and coordinating other patient care activities/services to provide the highest quality of patient care. Ability to work flexible hours as required meeting identified patient needs.
Medical Social Worker
(MSW)
Current Pennsylvania MSW license, one year of acute care experience and one year of home care experience. Possess strong assessment skills and knowledge of resources for homebound clients, re: PCA services, transportation arrangements, financial assistance available to indigence clients and placement eligibility and arrangements for clients who cannot manage at home safety.
