Our philosophy of care is patient-focused and client-oriented. Millenium Home Health Care's service is multidisciplinary in nature and based upon continuity of care and a case management process. Care is of the highest quality and aims to promote our patients' wellness and independence. For those to whom we provide both personal and professional care, we pledge to:

• Care for them and their families with quality home health care and home support services through a process that meets individualized health care needs and facilitates health and an optimal level of functioning;
   
• Recognize and respect the rights of patients and clients and their families to be involved in decisions regarding their home care services;
   
• Bring patients, clients and families a multidisciplinary base of services which may include: registered nurses, physical therapists, occupational therapists, speech pathologists, medical social workers and home health aides who will work in collaboration toward developing and implementing a home care plan;
   
• Communicate with the patient's physician, insurer and all others involved in the coordination of total home health care needs;
   
• Facilitate referrals to the necessary community agencies to meet patient's long-term needs.
